Care Manager (Registered Nurse)

The Care Manager utilizes a collaborative practice model to enhance the quality of patient care through effective management and monitoring of patient care programs.
    Key Responsibilities:
    * Conduct comprehensive patient assessments and develop individualized care plans.
    * Direct and evaluate clinical results and home environments for dialysis patients.
    * Provide vital education and training to patients and their caregivers.
    * Coordinate with a multidisciplinary team (Charge Nurses, Supervisors, and Physicians) to ensure continuity of care and safety.
    * Manage daily clinical operations in accordance with legal and regulatory requirements.
    Requirements:
    * Education: Associate or Baccalaureate degree in Nursing.
    * Licensure: Active Washington State Registered Nurse (RN) license.
    * Experience: At least one year of clinical nursing experience; previous dialysis experience is highly preferred.
    * Skills: Proficiency in Microsoft Office and clinical software; excellent communication and teaching skills.
    * Other: Valid WA driver’s license and proof of insurance for local travel.
